Forms
Students MUST submit the following:
1. a Physical or Health History Update Form (depending on the status of their physical)
AND
2. a sport-specific permission form
Physical Form
Required for participation in all sports, including tryouts
Must be signed by the child’s personal physician at the time of the child’s physical examination
This is the state-approved form; no other physical examination form will be accepted
Valid for one year from the date of the physical
Must be certified by our school physician before the child can participate in a sport; certification is done three times per year, prior to each sports season (fall, winter, spring)
Deadlines for submission are hard deadlines and cannot be missed
Health History Update Form
Required for participation in a Woodglen sport when the child has a valid Athletic Pre-Participation Physical Examination Form on file with the district which is more than 60 days old and less than 365 days old
Must be signed by the child’s parent or guardian; does not need to be signed by the child’s physician
Before completing this form, contact Diane Schneck, Woodglen School Nurse, to see if your child has a valid Athletic Pre-Participation Physical Examination Form on file with the district
Using this form without a valid and certified Athletic Pre-Participation Physical Examination Form on file with the district could result in missing a deadline and therefore having your child ineligible for athletic participation
