All Woodglen students who are planning to participate in any interscholastic or intramural sport during the school year are required to have a physical examination performed within 365 days prior to the first tryout or practice session.
Medical examinations for candidates for a school athletic squad or team are to be conducted at the student's medical home. As required by N.J.A.C. 6A:16-202(b), student medical examinations must be conducted at the medical home of the student and a full report must be sent to the school. A "medical home" is defined in N.J.A.C. 6A16-1.3 as meaning a health care provider and that provider's practice site chosen by the student's parent or guardian for the provision of health care.
As required by N.J.A.C. 6A16-202 (b), the medical report from the examining physician, nurse practitioner, clinical nurse specialist, or physician's assistant must include a determination concerning the student's participation. The school physician will provide this examination only for those students who do not have a medical home. This examination will be conducted at his/her office or other appropriately equipped facility.
What to Turn into the School
The State of New Jersey, Department of Education has issued an annual Athletic Pre-Participation Physical Examination Form
. These are the new State-approved documents. The physical examination results and the health history must be documented on these forms. The parent / guardian must complete the health history questionnaire. This questionnaire is to be reviewed by the child’s health care provider. The physical examination form is to be completed by the child’s health care provider and both forms must be submitted to the school prior to the deadline for the desired sport. Please remember that interscholastic and intramural soccer and field hockey will begin in September. Physicals must be completed by your child's health care provider and returned prior to the posted deadline.
